Your medical history includes:\n<ul>\n<li>Any medical conditions and illnesses<\/li>\n<li>Immunizations<\/li>\n<li>Allergies, reactions and sensitivities<\/li>\n<li>Past hospitalizations<\/li>\n<li>Other doctors treating you<\/li>\n<li>Medications and dietary supplements (i.e., vitamins, herbal products).<\/li>\n<\/ul>\n<\/li>\n<li>You and your doctor should agree about exactly what will be done during each step of your care.<\/li>\n<li>Know who will take care of you, how long treatment will last, and how you should feel.<\/li>\n<li>Ask to speak with other people who have undergone a procedure you are considering.
